*PRIMARY TASKS & DUTIES:* · Provide families and caregivers of children ages 0-5 with essential information about the significance of early childhood development (ECD) screenings.
· Guide families and caregivers through the screening process and offer necessary support during and after the screenings, especially when areas of concern are identified.
· Collaborate with external healthcare providers and facilities to ensure seamless continuity of patient care.
· Work closely with IT and Quality Nurse to accurately document and audit project and patient outcomes, maintaining detailed records of assessments, plans, interventions, and progress reports.
· Work closely with the Primary Care team and accompany parents or caregivers to well-child visits when appropriate.
· Screens for health-related social needs and provides resources for identified needs.
· Offer parent or caregiver education on child development, effective parenting techniques, and strategies to foster the child’s growth within the home environment.
· Connect families to appropriate resources and services, including early intervention programs, medical services, therapy services, educational programs, and social services, simplifying the navigation of complex service networks.
· Facilitate effective communication among providers to ensure a cohesive and coordinated effort to support the child’s development · Thoroughly complete post visit duties to ensure that all necessary tasks and documentation are finalized.
· Other duties as assigned.
*POSITION REQUIREMENTS:* _Education_ · Child development certificate or two-year degree from an accredited institution in early childhood education, or child development is preferred.
· High School Diploma required.
_Experience_ · Demonstrated success with clear thinking and ability to reorganize as needed.
· Demonstrated success in working independently, prioritization and problem solving.
· Demonstrated success in organization abilities.
· Demonstrated success in computer skills including ability to use computer for scheduling, word documents and reasonable keyboard skills.
· Demonstrated success in customer service/patient services or working with the general public, preferably in a medical care facility.
· Demonstrated success in managing difficult customer situations · Significant knowledge of medical practices and procedures in a primary care environment.
_Physical/ Environmental_ · Occasional lifting up to 50 lbs.

· Sitting, standing, and walking approximately 80% of the time · A medium to high level of manual dexterity required · Bending and reaching approximately 20% of the time · Normal accessibility and mobility throughout the region required · Normal overtime/extended work hours · Normal clinical working environment.
Requires good verbal and written communication skills.
Must be able to speak and read the English language.
· Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance.
· Work is primarily a facility based in an ambulatory care setting *ESSENTIAL FUNCTIONS/KEY COMPETENCIES:* · Read, write, and maintain patient records and related administrative documentation.
Ability to chart data within electronic medical records software.
